What is it?
An injury to the acromioclavicular (AC) joint, where the collarbone meets the shoulder blade — the ligaments holding the two bones together sprain, partly tear or fully tear, often from a fall onto the shoulder.
Separation vs dislocation

What is an AC joint separation?

The acromioclavicular (AC) joint is the small, mobile joint on top of the shoulder where the outer end of the collarbone (clavicle) meets the acromion, a bony projection of the shoulder blade. It is held in place by the AC ligaments around the joint itself and, more importantly, by the stronger coracoclavicular (CC) ligaments running from the clavicle down to the coracoid process of the shoulder blade. A direct fall onto the point of the shoulder — common in cycling, football, motorcycle and contact-sport falls — can sprain or tear these ligaments, letting the clavicle ride up and creating the visible “bump” or step-off deformity people notice.
This is genuinely a different injury from a shoulder dislocation, where the ball of the upper arm bone comes out of the shoulder socket. Surgeons grade AC injuries on the Rockwood scale from I (mild sprain) to VI (severe, rare displacement) — the grade, not the pain alone, decides whether surgery is needed.
Most Grade I–II injuries and many Grade III injuries heal well without surgery. Higher grades (IV–VI), and selected Grade III injuries in overhead athletes, throwers or manual labourers, benefit from surgical reconstruction of the CC ligaments — often done arthroscopy-assisted, as described on our arthroscopy surgery page. Grade I Sprain
A mild sprain of the AC ligaments with the joint still stable and no visible bump. Treated with a sling, ice and early physiotherapy — no surgery.
Grade II Sprain
A partial tear of the AC ligaments with the CC ligaments intact, giving mild joint looseness. Still usually managed non-operatively, with a short sling period and monitored physiotherapy.
Grade III (Borderline)

Grade IV–VI (Surgical)

Chronic AC Pain / Arthritis
An old, untreated separation or years of wear can leave a painful, arthritic AC joint. Distal clavicle excision or ligament reconstruction can relieve persistent top-of-shoulder pain.
Distal Clavicle Issues
Fractures near the AC joint and distal clavicle osteolysis (bone wear seen in weightlifters) can mimic a separation and need a careful, separate diagnosis and plan.

Assessment — examination and X-rays, often weighted or stress views, confirm the Rockwood grade and rule out fracture.
Anaesthesia — general or regional anaesthesia with a nerve block for comfortable post-operative pain relief.
Access — an arthroscopy-assisted approach through small portals, or a small open incision, depending on the injury pattern.
Reduction — the displaced clavicle is carefully brought back down level with the acromion.
CC ligament reconstruction — the torn coracoclavicular ligaments are reconstructed using a tendon graft, strong suture or a suture-button (tightrope) device.
Fixation — the button, graft or suture construct holds the clavicle reduced while the ligaments heal biologically.
Closure — small incisions are closed and the arm is supported in a sling.
Guided rehabilitation — a staged physiotherapy programme restores motion first, then strength, protecting the healing reconstruction.
