ACL Tear: Do You Really Need Surgery?

Not every torn ACL needs an operation. Here is how surgeons actually decide — and what happens if you choose either path.

The anterior cruciate ligament, or ACL, is one of the key stabilising ropes inside your knee. Tearing it — usually in a twist, pivot or awkward landing — is one of the most common serious sports injuries I treat. The first question almost every patient asks is the right one: do I actually need surgery? The honest answer is that it depends, and this article explains exactly what it depends on.

What the ACL does

The ACL stops your shin bone from sliding forward on your thigh bone and controls rotation when you change direction. When it tears, the knee can feel unstable — a sensation patients describe as the knee “giving way”, especially on turning or on uneven ground.

How a tear is diagnosed

Diagnosis combines your account of the injury (many people hear or feel a “pop”), a physical examination of the knee’s stability, and an MRI scan to confirm the tear and check for damage to the cartilage or meniscus at the same time.

Not all tears are equal

An ACL can be partially or completely torn, and the knee’s stability varies from person to person depending on the strength of the surrounding muscles. This is why two people with a similar-looking MRI can need very different treatment.

When surgery is usually recommended

Reconstruction — rebuilding the ligament using a graft, through keyhole (arthroscopic) surgery — is generally advised when:

  • You are young and active, or play sports that involve pivoting, jumping or cutting (football, basketball, badminton, kabaddi).
  • The knee gives way during everyday activities, not just sport.
  • There is an associated meniscus or cartilage tear that needs repair.
  • Your work or lifestyle depends on a fully stable knee.

When you can avoid surgery

A structured rehabilitation programme — without an operation — can work well for people who are less active, whose knee stays stable in daily life, or who are willing to modify the activities that provoke instability. Strong hamstrings and quadriceps can compensate for a great deal.

A realistic middle path: some patients start with rehabilitation and only move to surgery if the knee keeps giving way. Beginning with physiotherapy rarely makes a later reconstruction any harder.

The decision is not “surgery versus no surgery”. It is “which path gives this particular knee, and this particular person, the best long-term function”.

What reconstruction and recovery involve

ACL reconstruction is done arthroscopically through small incisions, usually as a day-case or overnight stay. Recovery is a commitment: expect around nine months to a year before returning to competitive pivoting sport, with physiotherapy the whole way through. Rushing back early is the single biggest cause of re-injury.

Why an untreated unstable knee matters

Repeated episodes of the knee giving way can damage the meniscus and cartilage over time, which raises the long-term risk of arthritis. That is why the decision is not only about today’s symptoms but about protecting the joint for the decades ahead.
