Best AC Joint Repair Surgeon in Ranchi
Bump on top of the shoulder?
AC joint repair.
The AC (acromioclavicular) joint is where the collarbone meets the tip of the shoulder blade. A fall onto the shoulder or onto an outstretched arm can sprain or “separate” this joint — a shoulder separation, which is a different injury from a shoulder dislocation. Many AC joint injuries heal without surgery; high-grade separations need ligament repair or reconstruction. Honest, grade-based AC joint treatment in Ranchi.

AC joint injury in Ranchi — quick answers
“A bump on the shoulder isn’t always a surgery.”
What is it?
An injury to the acromioclavicular (AC) joint, where the collarbone meets the shoulder blade — the ligaments holding the two bones together sprain, partly tear or fully tear, often from a fall onto the shoulder.
Separation vs dislocation
Not the same injury. A shoulder separation involves the small AC joint on top of the shoulder; a shoulder dislocation is the ball popping out of the shoulder socket. See the difference.
Does it need surgery?
Usually not. Grades I–II and most grade III injuries heal well with a sling and physiotherapy. Grades IV–VI, and select grade III cases, need ligament reconstruction.
Cost in Ranchi
Approx. ₹60,000–1.6 lakh when surgery is needed, depending on the fixation technique and implants used. Insurance & cashless available.
What is an AC joint separation?
“A separation, not a dislocation — the difference matters for treatment.”
The acromioclavicular (AC) joint is the small, mobile joint on top of the shoulder where the outer end of the collarbone (clavicle) meets the acromion, a bony projection of the shoulder blade. It is held in place by the AC ligaments around the joint itself and, more importantly, by the stronger coracoclavicular (CC) ligaments running from the clavicle down to the coracoid process of the shoulder blade. A direct fall onto the point of the shoulder — common in cycling, football, motorcycle and contact-sport falls — can sprain or tear these ligaments, letting the clavicle ride up and creating the visible “bump” or step-off deformity people notice.
This is genuinely a different injury from a shoulder dislocation, where the ball of the upper arm bone comes out of the shoulder socket. An AC separation stays painful and prominent at the very top of the shoulder, while a dislocation affects the whole joint and arm position — see our page on shoulder dislocation treatment for that distinction. Surgeons grade AC injuries on the Rockwood scale from I (mild sprain) to VI (severe, rare displacement) — the grade, not the pain alone, decides whether surgery is needed.
Most Grade I–II injuries and many Grade III injuries heal well without surgery. Higher grades (IV–VI), and selected Grade III injuries in overhead athletes, throwers or manual labourers, benefit from surgical reconstruction of the CC ligaments — often done arthroscopy-assisted, as described on our arthroscopy surgery page. Because the rotator cuff sits close by and can be strained in the same fall, it is also checked — see rotator cuff treatment.
AC joint grades & problems treated
Treatment is matched to the Rockwood grade and to you — not to the X-ray alone.
Grade I Sprain
A mild sprain of the AC ligaments with the joint still stable and no visible bump. Treated with a sling, ice and early physiotherapy — no surgery.
Grade II Sprain
A partial tear of the AC ligaments with the CC ligaments intact, giving mild joint looseness. Still usually managed non-operatively, with a short sling period and monitored physiotherapy.
Grade III (Borderline)
Both the AC and CC ligaments tear and the clavicle sits slightly high. Most heal well without surgery, but overhead athletes, throwers or heavy manual workers may be offered reconstruction if function suffers. See rotator cuff treatment for overlapping overhead-athlete injuries.
Grade IV–VI (Surgical)
Severe injuries — the clavicle buttonholed through muscle, grossly displaced, or trapped beneath the coracoid — need surgical reconstruction of the CC ligaments, often arthroscopy-assisted. See arthroscopy surgery.
Chronic AC Pain / Arthritis
An old, untreated separation or years of wear can leave a painful, arthritic AC joint. Distal clavicle excision or ligament reconstruction can relieve persistent top-of-shoulder pain.
Distal Clavicle Issues
Fractures near the AC joint and distal clavicle osteolysis (bone wear seen in weightlifters) can mimic a separation and need a careful, separate diagnosis and plan.
Does your AC joint
need surgery?
When it heals without surgery
- Grade I and II sprains — AC ligaments injured, CC ligaments intact
- Most Grade III injuries in non-overhead, lower-demand patients
- Treated with a sling for comfort, ice, and structured physiotherapy
- A cosmetic bump may remain, but the shoulder usually functions well
When surgery is needed
- Grade IV, V and VI injuries — significant or complete displacement
- Symptomatic Grade III in overhead athletes or manual workers
- Chronic pain, weakness or a “clunking” joint after conservative care fails
- Reconstruction of the coracoclavicular ligaments, with graft or suture-button fixation
Most AC joint injuries do not need an operation. Dr. Saurav grades the injury carefully on examination and X-ray, and recommends surgery only for the higher grades or when a lower grade genuinely fails to improve.
AC joint reconstruction,
step by step
Assessment — examination and X-rays, often weighted or stress views, confirm the Rockwood grade and rule out fracture.
Anaesthesia — general or regional anaesthesia with a nerve block for comfortable post-operative pain relief.
Access — an arthroscopy-assisted approach through small portals, or a small open incision, depending on the injury pattern.
Reduction — the displaced clavicle is carefully brought back down level with the acromion.
CC ligament reconstruction — the torn coracoclavicular ligaments are reconstructed using a tendon graft, strong suture or a suture-button (tightrope) device.
Fixation — the button, graft or suture construct holds the clavicle reduced while the ligaments heal biologically.
Closure — small incisions are closed and the arm is supported in a sling.
Guided rehabilitation — a staged physiotherapy programme restores motion first, then strength, protecting the healing reconstruction.
Surgery typically takes 60 to 90 minutes. The sling is usually worn for 4 to 6 weeks while the reconstructed ligaments mature, with physiotherapy starting early under guidance.

Preparing for AC joint repair in Ranchi
A correct grade, a clear plan, and careful technique give the best result.
Before your surgery
Before any decision about surgery, Dr. Saurav confirms the Rockwood grade with a careful examination and weighted or stress X-rays, and checks for associated injuries such as a rotator cuff strain or clavicle fracture. Blood tests and a fitness and anaesthesia assessment confirm you are ready, and you are told clearly which medicines to pause and when to stop eating before admission.
Because the choice between a sling-and-physiotherapy plan and surgical reconstruction genuinely changes outcomes, the reasoning behind the recommendation — and the alternative — is explained in plain language, with a transparent, itemised cost estimate and no hidden charges.
How your safety is protected
When surgery is needed, it is performed in a dedicated, sterile operating theatre with an experienced anaesthesia team and strict infection-control protocols. Arthroscopy-assisted technique keeps incisions small, reduces tissue disruption and lowers infection risk, while careful clavicle reduction protects the surrounding nerves and blood vessels.
Dr. Saurav personally guides your sling period and rehabilitation, and the Medifirst Hospital team in Ranchi stays reachable for any concern after you go home. Recommending the right treatment for the right grade — not surgery by default — is treated as the most important safety step of all.
